Stress and IVF: The Effect of a Mind-Body Program on IVF Success

The relationship between stress and infertility remains controversial. Despite that, a number of interventions have been developed to reduce stress, especially in IVF cycles, including Mind-Body (MB) programs, acupuncture and yoga. In fact RPMG is sponsoring a new session of Strong Yoga 4 Fertility beginning July 9th.

A study reported in the June 2011 issue of Fertility & Sterility demonstrates the benefit of a Mind-Body prorgam on IVF success. They ramdomized patients to participation in a Mind-Body program or a control group. Only nine percent of the MB group had completed a least half of the sessions prior to their first IVF cycle. Pregnancy rates were 43% for all subjects in the first cycle. 76% of the subjects had attended at least one-half of their sessions at cycle 2 start. Pregnancy rates for cycle 2 were 52% for MB participants and 20 for controls.

This demonstrates that MB participation was associated with increased pregnancy rates in cycle 2.

Octomom’s doctor’s license is revoked; Dr. Wisot has the last word

According to an article in today's LA Times:

The medical ethics controversy that erupted when Nadya Suleman gave birth to octuplets two years ago took a decisive turn Wednesday when the California medical board announced it will revoke the license of the Beverly Hills fertility doctor who helped the single mother of six conceive eight more children.

The "Octomom" case focused national attention on what critics have called "the Wild West" of fertility medicine. And Dr. Michael Kamrava, who assisted Suleman by implanting her with 12 embryos, became a symbol to some of the problems in the burgeoning industry.

Of course Dr. Wisot had the last word in the article:

"Dr. Arthur Wisot, fertility specialist with Reproductive Partners in Redondo Beach, said he believes the case may discourage doctors who previously might have been tempted to implant more than the recommended number of embryos.

"That's a good outcome," he said."

Reproductive Partners makes outreach to China

Two Reproductive Partners physicians have been invited to China to present the latest advances in fertility treatment and screening for genetic diseases at a major hospital in Bejing. Drs. Bill Yee and Greg Rosen  are currently lecturing on preimplantation genetic screening, molecular technology to screen for a variety of recessive genetic diseases and criteria for maximizing the success rate for elective single embryo transfer to reduce the chance of multiple pregnancy, including twins.

 

In conjunction with this outreach, the RPMG website has been translated and is now available in Mandarin Chinese. Portions of the website will shortly be translated into Japanese as Japanese-speaking staff is being added to the Redondo Beach location to better serve the Japanese-speaking community.

Reproductive Partners Medical Group, Inc. is pleased to announce that our website is available in Mandarin Chinese. Look for the box on the Home Page to link to the Chinese version.

There has been a lot of interest in IVF technology in China. Our doctors Rosen and Yee have been invited to lecture later this month in Beijing on the latest IVF technology. They will be concentrating on new PGS technology as well as improved cryopreservation techniques using vitrification. They will also be addressing preconception examination of an individual's genome for resessive genetic diseases.

Reproductive Partners' first four-session yoga for fertility workshop (STRONG Yoga™ 4 Fertility ) in February was a huge success. In fact, one of the participants in the initial program is now pregnant.

Studies have shown that alternative medicine techniques such as acupuncture and Mind-Body programs can improve the success rates for couples in fertility treatments such as in vitro fertilization.

The latest sessions have been scheduled on Saturdays, April 9th through April 30th from 2-4PM at the Beach Cities Health District, 514, N. Prospect, next to our Redondo Beach office. The cost is $240 and includes the course, Fertility Ball and course materials.

Dr. Wisot quoted in Life & Style

RPMG's Dr. Arthur Wisot is quoted in the April 11th issue of Life & Style reporting that Britney Spears is ready to expand her family. The medical issue is that she is on many psychtropic medications, reportedly Lithium, Valium, Ativan, Topomax and Straterra.

Dr. Wisot pointed out the importance of consulting her doctor before becoming pregnant on these drugs or stopping them cold. Women planning to conceive should have preconceptual counseling to discuss drugs they may be taking, other lifestyle issues that may need changing and genetic testing.

Fibroids and pregnancy-size does matter

 This is one of a series of news items from abstracts of studies presented at the Annual Meeting of the American Society for Reproductive Medicine as complied by Dr. David Meldrum, Scientific Director of Reproductive Partners. We appreciate the enormous amount of work it takes to compile and comment on these abstracts.


 Size does matter, at least according to two poster presentations at the meeting.

In one presentation, fibroid size was related to pregnancy complications. Fibroid size over 8 cm was associated with increased complications of  threatened abortion, and pain (43% had pain compared to 18% with fibroids under 8 cm). This high incidence of pregnancy complications argues for removal before pregnancy when fibroid size is over 8 cm. Pain during pregnancy is not uncommon even with smaller fibroids.

 

In a second poster presentation, the number of fibroids over 5 cm in size was associated with preterm delivery and the total number of fibroids was associated with arrest of labor.

 

In infertility the location of even smaller fibroids can be critical.

 

  The message here is that fibroids should be carefully evaluated for size and position before attempting pregnancy or fertility treatment .  

Does emotional stress affect the chances of getting pregnant?

No, according to a recent highly-publicized study in the British Medical Journal. They analyzed articles published from 1985 to March 2010 which reported a test of the association between pretreatment emotional distress (anxiety or depression) and pregnancy in women undergoing a single cycle of assisted reproductive technology. They concluded that the findings of this meta-analysis should reassure women and doctors that emotional distress caused by fertility problems or other life events concurring with treatment will not compromise the chance of becoming pregnant.

This is contrary to what most experts believe and our experience with stress reduction techniques such as acupuncture and Mind-Body programs improving IVF success rates, According to RPMG's Scientific Director, Dr. David Meldrum,   the discussion makes the point that because of the small percentage of significant stress in these studies (15%), any effect could have been diluted out by the 85% that did not have it. This could reconcile the finding that the mind/body program had a positive effect, because it would be those with more severe stress that would elect to be in such a study. Also most importantly (point also made in the discussion), stress reduction may allow couples to do more cycles (not examined in these studies), and that can have a profound effect of the chances of success. All studies of regions/countries where  IVF is covered have shown that the major reason for discontinuing treatment is being unable to cope with failure. It is our experience that most couples who discintinue treatment do not do so for financial reasons, but predominently because they cannot tolerate another failed cycle.

 We acknowledge that studies to date have not shown an impact of stress, but that patients who are under stress may still benefit  from stress-reduction, particularly if it allows them to do more cycles than they would otherwise  be able to tolerate psychologically .

Based on the high level of participation and enthusiasm of the participants, two additional sessions have been planned and are now registering participants.

For our Beverly Hills office, sessions have been scheduled on Sundays, March 20th through April 10th from 11:30 AM to 1:30 PM at our Beverly Hills office. For participants who prefer a South Bay location, sessions have been scheduled on Saturdays, April 9th through April 30th from 2-4PM at the Beach Cities Health District, 514, N. Prospect, next to our Redondo Beach office. The cost is $240 and includes the course, Fertility Ball and course materials.

Our 2010 pregnancy rates are now available

Because the type and age of patients treated may vary from program to program, the comparison of cycle statistics is complex. Couples should be aware of differences among programs as well as differences in specific characteristics when reviewing the data. There are a number of factors to keep in mind when reviewing data.

Comparison of programs is complicated. Only annual data should be quoted since short-term trends are unreliable. Outcomes should be reported clearly in terms of live births per cycle (clinical pregnancies for the latest year), retrieval or transfer. Success rates reported in the SART Data Registry format cannot be compared to programs that do not report to SART since non-reporters may utilize formats designed to inflate their success rates.

The following are our results as reported to the SART/CDC reporting system. In order to get a long-term view of our results we are presenting a cumulative report of the last five years as well as 2010, the latest reporting year (expressed in clinical pregnancies since not all the births have occurred and been reported). As entry criteria are highly variable for each program, and treatment processes vary, a center-by-center comparison of results may not be meaningful.
