"Peritubal adhesions" means that there is scar tissue outside but near the fallopian tube. They can affect fertility by distorting the tube, not allowing tubal peristalis to propel the sperm to meed the egg and for the egg to travel into the uterus or they can block the end of the tube not...

If it's a congenital absence of the vas then the option is IVF with aspirated sperm. If it is another type of blockage then surgery might be an option which may be covered by insurance. Talk to your urologist.

I don't know what the normal values are for your doctor's lab but in most labs it's 5-40 which would mean your is in the normal range and usually would not be treated.
