Low estrogens, more than 25 follicles, transfer cancelled

Post by Rikku » Fri Jan 08, 2016 9:35 am

Dear Drs,

I am very distressed right now. This is my first cycle and I am young and thin. I stimulated 12 days with puregon 150 units and developed 25 follicles, 8 of which more than 20 mm, several others around 18 mm and the remaining around 14 mm. I only have one ovary so this concerns that single ovary. My RE had the transfer cancelled because she feared OHSS. I have injected orgalutran the last 5 days and tomorrow I have to use Decapeptyl. The only problem is: my estrogen in only 206. My RE was very shocked because she was expecting over 4000. But she still canceled Ovitrel and transfer and is going for cryo. She said this maybe a sign of poor egg quality. Does this mean these are all just empty or defective eggs? :( Does this mean I should have went for fresh transfer anyway? Thank you

Sorry but I cannot make sense of all of this without complete information.

The complete information is: I am 31 years old and have BMI of 20. I have never had fertility treatments before. I have unexplained infertility of of less than one year. I only have on ovary because the other one was removed a long time ago because of a cyst. My doctor stared me with the scheme i described above and I had on the beginning of the cycle (day 4 of stimulus) an estrogen of 46. This value wasn't controlled until now. My follicles started growing steadily at a high number as I said above. My ER decides to cancel fresh ET because she was affeaid that i would hyperstimulate based on the striking ultrasound. Nonetheless she decided to check estrogen and it come only 206. Still we are doing retrieval and embryo freezing. I am only very scared that the follicles are all empty. How can there be more than 20 of them, the majority mature, if there is no estrogen? It must mean they are super defective. Sorry if I am not good at telling the history.

If the estradiol level is really that low, that would not be a good prognosis for the health of those eggs. But since your doctor decided to proceed with the retrieval you will just have to wait and see how it turns out. If the eggs are good, freezing the embryos and not doing a fresh transfer should not be detrimental to the embryos.

Hello, just a follow-up on this topic in case other women are struggling with the same problem. The estrogen was controlled once more on the decapeptyl shot day and was just slightly above 400. We got, from that one single ovary, 18 eggs which resulted in 15 embryos, all currently progressed to day 3.

Hello again, I am sorry to post again in this thread. I have got 8 day 5 blastocysts and 2 day 6 blastocysts. I would like to implant the 2 day 6 blastocysts, but my RE is pushing to implant only ONE. I do understand the risks of twin pregnancy and I accept them in exchange for higher chances of getting pregnant. What is you advice?

Thank you
